Incident and Causal Analysis/Corrective Action Management
Participants will study and practice the various methodologies and techniques for performing direct and root cause analysis. This course has been taught, on demand, at over 100 sites and facilities using the best management practices from both traditional and contemporary causal analysis processes. Given case scenarios and studies, participants will learn how to: select and use the appropriate analysis tool for a given situation including the Causal (WHY) Staircase, Barrier and Change Analysis and Task review analysis. An integrated methodology using a Comparative Timeline and the Even Conditions/Cause and Effect Tree will be discussed and practiced.
In addition, participants will understand the difference between behavioral issues and environmental worksite conditions that lead to errors, mistakes and failures. Using the SMARTS template, participants will learn and practice developing corrective actions that make a sustainable, measurable impact in the organization's performance.
